The Healthcare Ambassador is a pivotal member of the Landmark Interdisciplinary Team. This role actively engages with patients both in the home and by telephone.

The incumbent will use compassion and empathy to earn patients��? and caregivers��? trust, persistence to support and advocate for them, and excellent communication skills to build impactful relationships.

This position is full-time. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8 : 00am - 5 : 00pm.

It may be necessary, given the business need, to work occasional overtime.

Primary Responsibilities :

Conduct home visits and / or phone or video calls to patients to collect clinical and psychosocial information, and carry out designated tasks, including but not limited to :

• Resource referral and connection
• Quality measure documentation, support, and follow up
• Telemedicine facilitation and set up
• Collect information regarding patient conditions using standardized checklists and templates.
• Partner with Landmark interdisciplinary team (providers, nurses, social workers, etc.) to drive improvement in patient outcomes and satisfaction

Required Qualifications :

• High School Diploma / GED (or higher)
• 2+ years of customer service experience.
• 1+ years of clerical or office related experience
• Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
• Reside within 75 miles of Saint Louis, MO
• Required to have a dedicated work area established that is separated from other living areas and provides information privacy
• Ability to keep all company sensitive documents secure (if applicable)
• Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
• Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.
